Reappointment to the medical staff
Reappointment dates are based on the IDPH Credentialing Rules which assign reappointment dates to occur once every three years according to the last digit of your social security number.
Approximately three months before the expiration of initial appointment to the surgery center you will receive a pre-populated reappointment application form. You complete and sign the application, attach all required supplemental documentation, and mail it to our office.
Our authorized agent processes the application, queries the National Practitioner Data Bank, and obtains the necessary verifications.
Our Medical Director will complete a Clinical Competence Assessment profile based on the following data:
- Pertinent occurrence reports and complications;
- Patient, staff and physician complaints;
- Post-op hospital admissions;
- Post-op infections;
- Tissue discrepancies;
- Results of any pertinent performance improvement activities or QI findings;
- Successful attainment of board certification since last appointment, if applicable.
- National Practitioner Data Bank reports
Your reappointment application is reviewed and approved by the center’s Credentials Committee, and you are notified of your reappointment to the Medical Staff.
In order to remain active on our medical staff, you must perform the equivalent of 3 cases per year - a total of 9 cases. If you perform the required number of cases prior to this date, there is no reappointment fee.
If you have performed less than the equivalent of three (3) cases per year within the reappointment period, you must submit a surgical case listing for the most recent 12-month period from the hospital/facility where you performed the majority of your cases, a letter from a Hospital Department Chairperson stating you are a member in good standing, and a reappointment processing fee of $250.
